Do I Need a Landlord Verification Form if I Have a Signed Lease?
No. If you have a current lease signed and dated by both you and your landlord, it fulfills the state’s housing verification requirement completely. You do not need to fill out the alternative Landlord Verification form.
The Open Card Rule & Legal Citation
Under Oregon Administrative Rule OAR 410-120-2010(5)(d), the state mandates the collection of “income and lease/agreement verification” for Health-Related Social Needs (HRSN) rent assistance. A formal, signed lease perfectly serves as this baseline legal proof for your application packet.
The HRSN Verification of Landlord/Tenant Relationship and Rent Owed form is simply an alternative backup designed by the Oregon Health Authority (OHA). It is only required for “doubled-up” or informal renters (such as people living with family or friends) who do not possess a formal written lease agreement.
Who Qualifies for Open Card HRSN Rent Assistance?
To qualify for the program through our agency, you must meet the following criteria:
- Active OHP Open Card Coverage: You must be covered under the Oregon Health Plan’s fee-for-service (Open Card) model.
- Qualifying Life Transition: You must meet specific clinical risk factors, such as transitioning out of homelessness, exiting institutional care, or facing a high risk of eviction due to circumstances that impact your health.
How Long Does the State Approval Process Take?
Once our outreach specialists audit your paperwork and verify it is 100% complete, your packet is fast-tracked directly to Acentra Health (the state’s primary case management contractor for Open Card members). The official state review window typically takes up to 30 days. Submitting a flawless, fully signed lease on day one is the best way to prevent processing delays.
